Persistent or recurrent urinary infection is in some patients associated with the development of localized scarring of one or both kidneys, attended by hypertension in some cases, and progressive renal insufficiency especially when there is obstruction.
Note :
In these circumstances, chronic pyelonephritis is a reasonable diagnosis. Scarred kidneys associated with hypertension or renal failure are also found in patients with no history of urinary infection; here, the diagnosis of chronic pyelonephritis is more in doubt. In children, the correlates of chronic pyelonephritis include vesico-ureteric reflux and more rapid renal scarring. In adults, a high intake of analgesics appears to predispose to pyelonephritis or may lead independently to chronic renal failure.
