A manifestation of subacute bacterial endocarditis seen most frequently in patients with Streptococcus viridans endocarditis, consequent on the embolization of glomerular capillaries by fragments of vegetations dislodged from the cardiac valves.
Note :
The lesions are focal and the affected glomeruli may show complete infarction or a cellular proliferation leading to fibrosis involving a portion of the tufts. May be associated with segmental infarcts of the kidney substance. Clinically manifested by intermittent haematuria (often microscopic), proteinuria, lumbar pain, and little impairment of renal function. This condition should be distinguished from the immune complex glomerulonephritis of bacterial endocarditis which is a diffuse proliferative glomerulonephritis that can lead to severe renal impairment.
